We’ve partnered with Phreesia and Microsoft Teams, our new telehealth platform, to offer you the very best option for safe and convenient care. Our providers can diagnose, treat, educate, and prescribe via telehealth appointments. Both new and current patients can take advantage of these services all from the comfort of their own home.

How do I schedule a telehealth appointment?
All of our specialties offer Teams for Telehealth appointments and they work for both current and new patients.
Call our office at 920-725-9373 and one of our staff can help you with scheduling and instructions. We want to make the process simple for you and suggest you read through these Frequently Asked Questions to make sure you understand the steps involved. We’re always here to help so reach via phone if something isn’t clear.
How do I prepare for my telehealth appointment?

1. Install Microsoft Teams on your smartphone, iPad, or tablet device. If you are using a computer or laptop, there is nothing to install. *see next question for instructions for apple and android devices

2. To confirm your appointment and complete your pre-registration, you will receive a text and email 3 days prior to your appointment date.

3. Confirm the date, time, and your provider information. Click “Confirm appointment”.

5. Once you have confirmed, you will need to verify who you are, review your information, and possibly make a payment. Click “Get Started”.

6. Identity verification includes your date of birth and address.

7. If using a computer for your telehealth appointment, you will need a “security code” which will be sent via text or by voice to the phone number you have listed. To access your “security code”, simply type the telephone number you have listed into the grey box and click “Send Security Code”.

*The two phone numbers must match to receive your security code*

8. After, you will be asked to enter your “security code”. Click Verify.

9. Please provide your phone number and email so we are able to contact you if issues arise during your visit. Click “Continue”.


10. If you have not already, please download Microsoft Teams on your device.

 Note: You do not need a Microsoft Account and no additional apps need to be downloaded for your telehealth visit.

How do I install Microsoft Teams? (Apple & Android Devices)

*Rest assured, Microsoft Telehealth Teams is HIPPA compliant and secure*

Install on your computer or laptop:

You do not need to install Microsoft Teams on your computer or laptop. All you have to do is click on the link in your email.

Install on your mobile device:

Instructions for Apple:

1. Make sure to have your Apple ID and password handy, you will need to enter this to be able to download from the app store

2. Search for “Microsoft Teams” in your app store.

3. Select “Get” and sign in with your Apple ID.













Instructions for Android:

1. Make sure to have your Google Play username and password handy, you may need to enter this to be able to download from the app store

2. Search for “Microsoft Teams” in your app store.

3. Select “install”.



How do I start my telehealth appointment?

1. Click the telehealth link from the text or email you received.










2. Complete your Telehealth appointment Check-In. 

3. Click the orange box that says “Start Telehealth Session”

4. Allow camera and microphone sharing.












5. You will be ready for your live and interactive appointment with your provider.

What to expect during a Teams for Telehealth video visit?

Our telehealth appointments are designed to be similar to that of a traditional office visit, but from the comfort of your home. Your provider will meet with you face to face and discuss your concerns. They can order tests and send prescriptions to your preferred pharmacy. Our goal is to answer all your questions and give you next steps in your care plan.

How is my telehealth appointment billed?
  • Your insurance company views a telehealth appointment (video + audio) the same as an in-person office visit.
  • A telehealth appointment (video + audio) is billed the same as an in-person visit and counts toward meeting your deductible.
  • Some insurance companies require a copay for telehealth appointments (video + audio) and some do not.
  • Talk with your insurance company for further information.
Is Teams for Telehealth secure?
We’ve teamed up with Phreesia and Microsoft Teams to provide you with industry-leading privacy and security. Patient data is safe guarded and the whole process is HIPAA secure.
Can I have a remote family member(s) join my telehealth appointment?
Yes! Just let our schedulers know that you want your family to join the appointment. Provide them with your family member’s email address and we will forward the appointment link to them.
Are translation services available for my appointment?

Yes! When scheduling your appointment, please let our staff know you will need translation services and we will forward the appointment link to them. 

What if I get disconnected during my telehealth visit?
  • If you get disconnected from your telehealth visit, simply go to your email and look for the one that says “Your Telehealth Session Link” to reestablish the connection with your provider.
  • If that does not work, our provider will call you and the visit will be via telephone.
What is the "lobby" in Teams for telehealth?
If your provider is meeting with another patient and isn’t available at your appointment time, you will receive a pop up notice that you are in the “lobby”. The concept is the same as if you were in the doctor’s office. They will begin your video visit shortly.
